You did everything right. You went to bed at a reasonable hour, you cut the second glass of wine, you took a real weekend, you even started saying no to the things that drain you. And you are still tired. Not "I could use a nap" tired. More like you are dragging your own body through the day and hoping nobody notices.
Here is the thing about that kind of exhaustion: when fatigue survives good sleep and genuine stress management, it stops being a lifestyle problem and starts being a signal. Something upstream is out of tune, and no amount of green juice or discipline is going to out-hustle it.
Over 15 years in practice, I have watched more patients than I can count get told some version of "you're just stressed, you're a busy person, this is normal." Sometimes that is true. Often it is not. And the difference matters, because one of those situations resolves with rest and the other one necessitates a blood draw.
Let's sort out which is which.
Everyday tired versus something-is-wrong tired
Normal fatigue has a cause and a cure. You had a brutal week, a red-eye flight, a sick kid, a deadline. You feel it, you recover, you bounce back within a day or two of rest. That is your body doing exactly what it should.
The fatigue I want you to pay attention to is different. It has a few tells:

For women, perimenopause can start in the early-to-mid 40s, sometimes sooner, and the fluctuations, not just the eventual decline, are what wreck your energy and sleep. Low estrogen and swinging progesterone disrupt the deep, restorative stages of sleep, so you can be in bed eight hours and still wake up unrefreshed. Add night sweats and 3 AM wakeups, and fatigue becomes almost inevitable. These are core hormone imbalance symptoms, and they are some of the most under-recognized drivers of "I'm exhausted and nobody can tell me why."

A good workup rules these in or out. Anyone who jumps straight to hormones without considering the rest isn't doing a full evaluation.
When to test, and what to actually check
Here's my rule of thumb: if fatigue has lasted more than a few weeks despite adequate sleep and reasonable stress management, or if it comes with any of that supporting cast of symptoms, it's time for hormone testing and a broader workup. You don't have to wait until you're desperate.
If you were sitting across from me, here's roughly where I'd start with labs:
- Thyroid, done properly. Not just TSH. I want a fuller picture: free T4, free T3, and thyroid antibodies when the story warrants it.
- Iron studies. Ferritin especially, plus a complete blood count. Ferritin tells you your stores, and stores drop before anemia shows up.
- Metabolic basics. Fasting glucose and hemoglobin A1c, because blood sugar swings masquerade as fatigue constantly.
- Vitamin D and B12. Cheap, common, fixable.
- Sex hormones, interpreted in context. For women, this means understanding where you are in your cycle or menopause transition, because a single random estrogen level tells you very little on its own. For men, a morning testosterone level is the anchor.
- Cortisol, when the pattern suggests it, measured with timing that respects its daily rhythm.
The interpretation matters as much as the numbers. A result inside the "normal range" isn't automatically optimal for you, and a snapshot of a fluctuating hormone can mislead if nobody accounts for the timing. This is why hormone testing is a starting point for a conversation, not a verdict handed down by a lab printout.
Where to start
If you're overwhelmed, start with the highest-impact, lowest-friction step: get the foundational labs drawn. Thyroid, iron, metabolic markers, and vitamin levels are inexpensive, widely available, and they catch a surprising share of what's making people feel terrible. That's your week-one move.
While you wait on results, tighten the fundamentals, not as a cure, but so you're not confusing a fixable lifestyle gap with a medical one. Protein at breakfast to steady blood sugar. Morning daylight to anchor your cortisol rhythm. A consistent sleep and wake time. Movement you'll actually repeat. These won't override a thyroid problem or low estrogen, but they clear the noise so the real signal is easier to read.
Then bring the results to someone who will interpret them in the full context of your symptoms, your age, and your life, not someone who glances at "normal" and sends you home still exhausted. One lever at a time. You don't fix five circuits in one week, and you don't need to.
